Bellow you will find the steps on how to add a new Assessment or make modifications to an existing Assessment.

Add a New Assessment
- Hover over the Managers tab in the navigation bar and click Client Manager from the drop-down.
- Open appropriate Client.
- Click Assessments from the Notes/Information section on the menu on the left.
- Select Assessments from the Select Category drop-down. The drop-down menu below will populate with Select Assessment.
- Click the Select Assessment drop-down, and select the type of Assessment needed. Once the appropriate Assessment is selected, you will be automatically directed that specific Assessment form.
- Fill in the form as thoroughly as possible.
- If you are unable to finish the Assessment in one sitting, scroll to the bottom of the page and select Save Progress. You can then finish the Assessment at a later time by returning to the Client File and clicking on the Assessments tab, and open that Assessment to complete.
- Once you have completed the Assessment, click the green Mark Completed and Sign button at the bottom of the page.
Note: Once an Assessment is completed, you will be able to view the Assessment, however, all fields will be disabled for modifications.
Edit an Assessment
In order to make modifications to an existing Assessment, you will need to unlock the Assessment from either the Client View or from the Assessment.
- Locate the Assessment(s) you would like to modify.
- The Lock Status category will show if the Assessment is locked or unlocked. If the Assessment you need to modify is locked, click the Closed Lock icon to unlock it.
Note: Assessment can also be unlocked from within the Assessment, simply scroll to the bottom of the Assessment and locate the Lock Icon.
- Click the Folder icon to access the Assessment.
- Make any needed modifications, then scroll to the bottom and click Mark Completed and Sign. This will lock the assessment.
Note: Once an Assessment in unlocked, you can modify all fields except for the previous signatures.
- All actions taken to the Assessment are captured in the Assessment History. The Assessment History is located at the top of the Assessment just below the Assessment title.
