You can add your Signature in the Patient Portal. When you do you this, you will create a PIN which will be used as authorization when you want to apply your Signature to a document in the Client Portal.

Add Client Signature
- Navigate to https://myhealth.sharenote.com/
- On dashboard, click Create Signature.
- Add Signature using your mouse or touch pad.
- Create and Confirm your PIN.
- Select a Security Question.
- Enter and Confirm your Security Answer.
- Click Save Signature and Security Information.
Update Your Signature
- Navigate to https://myhealth.sharenote.com/
- On dashboard, click Update Signature.
- Enter PIN and click Click to Verify button.
- Follow steps above to update Signature.
Add Parent/Guardian Signature
If the Client is 18 years of age or younger, you can add the signature of a parent or guardian.
- From the dashboard, click Create Signature.
Note: As soon as the Client is older than the age of 18, the option to add a Parent/Guardian Signature will disappear.
- Add Signature.
- Enter Parent's First and Last Name.
- Enter and confirm the PIN.
- Use the drop-down to Select Security Question.
- Enter and Confirm the Security Answer.
- Click Save Signature and Security Information.
