This process is typically for Self-Directed Services and contains two parts. Supervisors and Administrators create Goals for the Staff Member and then use Staff Plan Notes to recorded Staff Review.

Access Staff Manager
- Hover over the Managers tab in the navigation bar, and select Staff Manager from the drop-down.
- Click on the appropriate Staff Members name.
Add Staff Goal
- Click Goals tab.
- Click Click here to add a new Goal, and Add New Goal window will display.
- Enter Goal Title and Full Description/Purpose.
- Select Status.
- Click on Add Goal.
Add Staff Plan Notes
- Click Staff Plan Notes tab.
- Click Add Staff Plan Note and Staff Plan Note screen will display.
- Staff Name defaults.
- Enter Date Implemented or Reviewed/Plan Date.
- Select Contact Type (optional).
- Enter Total # of Hours.
- Enter Review of Staff Services.
- Select Goal to evaluate.
Note: Select Check All to select all Goals.
- Select Clinical Skills if applicable.
- Enter Training Needs response.
- Enter Ongoing Development Areas response.
- Enter Required Frequency of Supervision response.
- Enter Supervisor's Notes/Supervision Summary response if applicable.
- Attach Files if applicable.
Sign & Save Staff Plan Note
- Select I Agree to the terms of the Attestation Statement checkbox.
- Enter Signature PIN (number).
- Select Set Signature Date as.
- Click Save Staff Plan Note.
