Generate the CMS 1500 Claim Form (ShareNote)

  Permissions: You must have Billing Permissions.

The information that is populated in your CMS 1500 Claim Form is pulled from various parts of ShareNote Community such as the Billing Profile, Staff, and Client set up.

1. Hover over the Managers tab in the navigation bar, scroll down to Direct Billing, then select Transaction History from the drop-down. 
2. To generate the form for a specific claim, enter in either the date range or Transaction ID.
   
   
3. Click View Transactions or Search. 
   
4. Click View to view the claims housed inside the transaction.
   
   
   
   
5. Click Generate CMS 1500, and the PDF form will open in a new tab.
