Once the EPCS Coordinator has activated your Provider Identity Proofing Process, these are the steps you will need to follow to prove your identity.
Note: None of the information given during the Identity Proofing process is stored by ShareNote Community or DoseSpot, nor are any charges placed on the credit card. This information is only used to be shared with Experian to verify that the Provider is who they say they are. This is done for the Provider's protection from fraudulent activity.
Access Identity Proofing Form
- Log in to ShareNote Community.
- Hover over the Managers tab in the Navigation bar, and select Client Manager from the drop-down.
- Select any Client.
- Select Electronic Prescriptions from the Health section on the menu on the left.
- Click the green Open Electronic Prescription System button.
Note: If the green button doesn't appear, be sure the following Client Information is entered and try again. First Name, Last Name, Home Address (including City, State, and Zip Code), Home Phone Number, and Date of Birth.
Complete Identity Proofing Form
- In blue window, click the orange Identity Proofing Agreement button.
- Confirm Provider's information in new window.
- Select the Acknowledgement checkbox if he/she agrees to it.
- Fill out the form. The form asks for personal information, including the Provider's Social Security Number and a credit card number.
IMPORTANT: Ensure that the Phone Type is set to Mobile, that your current phone number is correct (numbers only, no parenthesis, periods, or dashes. e.g. 77098555631 ), and that the This is My Mobile Number checkbox is selected. - Click Next.
One-Time Use Password
- You will receive a text message at the provided number with a one-time use password. This password will expire in seven days if not used.
- Enter the one-time use password back in ShareNote Community browser.
- Click Next.
Identity Verification Questions
- Experian window will open.
- Experian will ask three or four questions to be answered by the Provider to prove they are who they say they are.
- Click Next.
Confirmation & Set PIN
- You should receive a success verification.
- Click the Set PIN link below the verification message.
- Create a four-digit PIN.
Note: Remember this PIN as you will need to use it each time you send a Controlled Substance electronically. - Click Ok.
The Provider has now completed the first step of Identity Proofing. You will receive an Identity Proofing letter in the mail to confirm the application process. When you have been approved the EPCS Coordinator will need to initiate two factor authentication (TFA) for the Provider.
